Patient Forms, Map and Directions to our Office

Your first visit to our practice establishes a vital foundation for our relationship with you. During the first appointment, you will spend approximately 2 hours with Dr. Creech. We make sure to obtain important background information, like your medical history and a list of medications that you may be taking, as well as a report of any dental problems that you may be having.  Following a thorough head and neck exam, we will obtain any radiographs that may be needed for your specific case. Once a complete soft tissue exam, oral cancer screening, gum health and dental examination has been completed, a treatment plan will be generated presented with recommendations and options to meet your individual oral health needs and goals. To obtain the best treatment possible, our recommendations may include referrals to dental specialists if necessary. If you are satisfied with the treatment recommendations and there is time remaining, you may choose to get your teeth cleaned at this initial appointment. To understand more of what to expect for your first visit to our practice, please read through this page.
